Palliative CareThe Palliative Care Program at Southeastern Med is in place to provide comfort and dignity for anyone suffering with an advanced or chronic illness.

Palliative care requires a team approach that aims to relieve symptoms and improve quality of life for you and your family. In addition to medical treatments, palliative care educates you about your chronic disease or condition. Understanding the disease and how it affects you physically and mentally is vital to managing and controlling your symptoms.

Managing Your Illness

Illnesses and chronic diseases can last months, sometimes even years, and require expensive medical treatments and multiple hospitalizations. One of the goals of our Palliative Care Program is the early management of these conditions with a blend of pharmacology, relaxation techniques, guided imagery and behavioral interventions. Many illnesses can be effectively managed as outpatient procedures, controlling symptoms before they require hospitalization and decreasing the financial burden on you and your family.

Palliative Care Team

Our team includes nurses, social workers, chaplains, respiratory therapists, pharmacists and a physician advisor. Case managers coordinate care between home health, hospice or extended care facilities, allowing us to focus on you as a whole individual with a mind, body and soul.

Palliative Care Isn't the End

Although palliative care can include end-of-life issues, such as understanding living wills, health care power of attorney, organ donations, resuscitation techniques and nutritional concerns, the goal of the program is to bring you back to wellness. Through support and education, you and your family will learn about your choices and what you're comfortable with doing medically as the disease progresses. Coping with a chronic illness can be difficult and confusing, but you and your family don't have to go through it alone.
